• Regence Blue Cross Blue Shield

We are in the processes of becoming a contracted provider with certain OHP’s CCOs and may be able to bill insurance with a prior authorization.

As the patient, you are responsible for understanding your benefits and for any services not covered by your plan. See Insurance FAQ link at the bottom of the page for questions to ask your insurance provider regarding your nutrition benefits. If you have questions, let us know!

If you have out-of-network benefits, we can provide you with a “superbill” that you can submit to insurance to seek partial out-of-network provider reimbursement. A superbill is essentially a receipt of the services with the appropriate ICD-10 codes provided by a healthcare practitioner.

Self-payment is also an option.

Initial Consultation Rate:

• 60 minutes: $185

Follow-Up Appointments:

• 30 minutes: $75

• 60 minutes: $145

*These rates are for cash pay only. These are discounted contracted rates and are only offered when paid in full the day of the appointment.

Discounted package deals are available.

• Self-pay rates are due on the day of service.

• For insurance billing, we will bill your insurance our contracted rate. You will be responsible for your co-pay, co-insurance and/or part of your deductible if it hasn’t been reached; or any services not covered by insurance.

• A credit card will be held on file and charged the day of service. All major credit cards accepted as well as HSA/FSA cards.

• We know that life happens! If you need to cancel or reschedule an appointment, please gives us at least 24-48 hours notice.

  • For appointments cancelled the same day a $50 fee will be charged to your credit card on file, with the exception of an illness or extenuating circumstances.

  • For no-shows, you will be responsible for the regular charge of the appointment which is not covered by insurance.
